All students enrolled in Holy Redeemer Catholic School must have a completed Emergency Medical Form on file at the school. It is an obligation of the parent/guardians to keep the information on this form current.

ALL APPLICATION FORMS & STUDENT HEALTH FORMS MUST BE SUBMITTED TO TADS

PRIOR TO THE FIRST DAY OF SCHOOL.
